Angie Vanderzeil
Angie is a passionate physiotherapist who is dedicated to providing high-quality and patient-centred care. A strong communicator, she will explain your situation and work with you to create a treatment plan that will work for you. She will aim to empower you with knowledge and skills to help you optimise your health.
As a strong advocate for exercise, she will ensure to provide you with an exercise program that suits your individual abilities, preferences and goals.

Please ensure that you are wearing appropriate clothing to allow free movement and a full assessment of the body area in question. Shorts or tights can be a good option. Having a couple of clean bath towels handy for use during the assessment can also be helpful. Apart from that, we will bring any necessary equipment.

Private Health Insurance
If you have Physiotherapy extras cover on your private health insurance policy, you may be eligible to claim a rebate on your appointment. Please check with your insurer directly prior to booking an appointment, as cover varies. Unfortunately, we are not able to claim rebates for you. Payment is required in full on the day of service. We will provide a receipt for you, which you can use to retrospectively make a claim with your insurer.
TAC, WorkSafe & DVA
Currently, we are not registered to provide services to those funded by TAC claims, WorkSafe Victoria claims, or DVA card holders. We apologise for this inconvenience.
Medicare
If you wish to claim rebates against a Medicare-funded GP Chronic Condition Management Plan (GPCCMP), you will require a written referral to Good Move Physio Co from your GP / family doctor. You will be required to pay a gap payment of the balance of the appointment fee on the day of service. You may be eligible for up to 5 subsidised sessions per year. Please advise on booking if you would like to use a GPCCM.
Bookings
Once you have received an appointment confirmation via phone call, text or email, your appointment is confirmed. You will then be subject to the below conditions regarding payment, cancellations or no-shows.
Changes and Cancellations
Changes or cancellations to appointments are accepted via phone call, text or email. Please provide at least 24 hours notice when cancelling an appointment. Cancellation within 24 hours of the appointment time may incur a Late Cancellation Fee of up to 25% of the appointment fee, depending on the circumstances.
Late Arrivals
Please ensure to be at the specified location for your appointment on time. If you are late to your appointment, please let us know with as much notice as possible. Your available face-to-face time may be reduced accordingly. This is to enable subsequent appointments with other patients to occur as scheduled.
No-Shows
If you are not at the specified location for your appointment time, and have not provided notice, we will wait for you for 15 minutes. If you have not arrived within 15 minutes, your appointment will be cancelled and you will be charged a Did Not Attend Fee of up to 50% of the appointment fee. Fees may be waived at our discretion for genuine emergencies or exceptional circumstances.
Fees
Fees and durations for different appointment types are listed on our website. Please note that the face-to-face time may be slightly shorted than specified at times. This is to reflect any additional tasks required, such as completing legally required notes, or preparing other required documentation such as correspondence, reports, referrals, home exercise programs or similar. Any changes to fees will be updated on our website. Please check for updates prior to booking your appointment.
Payment is required on the day of service. Payment is preferred via card and we accept Visa, Mastercard, AmEx and EFTPOS payments. A 1.1% card surcharge applies per transaction. Bank transfer payments are also accepted.
If your account is in arrears, you will not be able to confirm any future appointments until all outstanding invoices are paid. If you will not be able to pay on the day of service, please discuss this prior to booking your appointment.
Pre-payment for multi-visit passes is required in full. Pre-paid visits are valid for 18 months from the date of payment.

Definitions
‘You/your’ refers to the person making the booking, for themselves or on behalf of another person.
‘We/us/staff/your treating physio’ refers to those representing Good Move Physio Co
‘Treatment/visit’ refers to clinical services provided by Good Move Physio Co
By confirming an appointment, you consent to the terms & conditions as outlined below.
Telehealth
If booking a telehealth appointment, you accept that telehealth has limitations when compared to face-to-face care and acknowledge that you are accepting of those limitations and risks. You are also responsible for ensuring that you have the required equipment, such as a smart phone, tablet or computer with internet connection, and the required software. Good Move Physio Co is not responsible for any technical failures that affect your appointment or telehealth experience.
Patient Responsibility
You agree to provide accurate information, follow treatment instructions and notify your treating physio of any adverse effects or changes in condition. You indemnify Good Move Physio Co for losses arising from your failure to provide accurate information or follow instructions.
Respectful Behaviour
When dealing with Good Move Physio Co, you will be treated with respect at all times. We require that you treat us with respect in return. If your behaviour is deemed at any time to be disrespectful, inappropriate, demeaning, abusive, aggressive or threatening; your involvement with Good Move Physio Co will be terminated and all future appointments will be cancelled.
Staff Safety
Our staff have a right to be and feel safe while at work. We reserve the right to withdraw services if there are any circumstances which may affect the safety of our staff. This may include behaviours or risks associated with you, members or visitors of your household, pets, neighbours, or the physical environment of your home. It is your responsibility to report any possible risks, so they can be managed appropriately. Please refrain from smoking indoors within 2 hours of your appointment.
Infection Control
At Good Move Physio Co, we take infection control seriously. We request that you inform us of any symptoms of possibly infectious illness (for example, symptoms of cold/flu, gastro or Covid) if they arise prior to any confirmed appointments. This is to ensure that illnesses are not spread between staff and other patients, especially those at high risk of severe illness. Your appointment may be postponed or converted to telehealth, depending on the circumstances.
Refusal of Treatment
We reserve the right to refuse to provide requested interventions if necessary. This may occur when they are deemed to be inappropriate, contraindicated, high-risk, or outside the scope of practice, training, or ability of your treating physio. If this is the case, the risks and benefits of different treatment options will be discussed with you and alternative interventions will be offered. A referral to another practitioner who is able to provide that intervention (if appropriate) will also be offered.
Urgent & Emergency Situations
In a situation that your treating therapist deems to be urgent, you give us your consent to communicate with other relevant parties such as your doctor or next of kin, in order to protect your health and safety. In a situation that your treating therapist deems to be an emergency, you give us your consent to call an ambulance to your home or appointment location. Communication
By accepting these terms & conditions, you agree to being contacted via the contact methods given during your registration. This will primarily be for the purposes of booking, confirming appointments, appointment reminders, payment reminders, provision of receipts and other documents, and follow-up after appointments. This may also occasionally include marketing material. You may opt out of marketing material at any time by contacting us and making this request.
Complaints
Please contact us if you have a concern or complaint. We will discuss the matter with you and attempt to find a resolution. Changes to Terms & Conditions
We may vary these Terms & Conditions from time to time. Updated Terms will be published on our website and communicated to patients where reasonably practicable. Continued use of services after changes are published constitutes acceptance of the updated Terms & Conditions.

Your information is also used in the administrative processes required to deliver this care (For example: for communication, booking and billing processes). Your information may also be used occasionally for other internal processes such as accreditation, quality assurance, internal audits or marketing purposes. We may disclose information about you to others outside of our practice as permitted or required under law. This will include situations where we disclose information about you to:

• Consult with other health professionals involved in your care

• Lessen or prevent a serious threat to a patient’s life, health or safety or a serious threat to public health or safety
• Liaise with a medico-legal expert, insurer, or legal practitioner for the purpose of reporting adverse incidents, legal proceedings, or for obtaining legal advice
• Best carry out your care. For example, to a parent, guardian, next of kin or nominated emergency contact when required.
Your information will never be provided to unnecessary third parties without your express prior consent. Your information will never be sold to any third parties.
We are committed to storing your information securely. Measures to uphold your privacy inclue:
• Ensuring hard copies of documents are stored in a secure location
• Destroying hard copies of documents before disposal
• Holding information on password-protected computers with encrypted hard drives
• Storing information on encrypted and password-protected mobile devices
Holding information in secure third-party software that is compliant with Australian Privacy laws and utilises Australian-based servers for data storage
